How to fill in your W-9 Form correctly
A. If your business is registered with the IRS as a Company:
Enter the business name in line 1 exactly as registered with the IRS (leave line 2 blank!)
In Part I, fill in the employer identification number (leave the social security number blank)
B. If your business is registered with the IRS as an Individual:
Enter your name in line 1 (leave line 2 blank)
In Part I, fill in the social security number (leave the employer identification number blank)
